Elephantiasis nostras verrucosa (ENV) classically occurs in the setting of chronic nonfilarial lymphedema. Prolonged lymphostasis of the affected extremity eventually leads to accumulation of protein within the interstitial fluid, which in turn induces proliferation of fibroblasts.
